About Crestwood
Since the Minassians opened the doors of Crestwood Nursing & Rehabilitation Center in 1978, generations of the family have continued to own and operate the center under the same guiding philosophy: to provide the highest quality care to residents in a home-like setting.
Over the years, we have continued to transform our Warren, Rhode Island, location to accommodate the evolving clinical, social, physical and spiritual needs of residents. But, one thing hasn’t changed – our commitment, compassion and care. Today, we offer 76 private and semi-private rooms, skilled nursing services, a range of therapies, hospice, a 220-plus square foot rehabilitation center, and more. We are a top-rated medicaid and medicare facility. However, if you don't qualify for either, we have a reasonable private pay rate.
At Crestwood, we are focused on providing your loved one with a superior quality of life and offer numerous activities and social programs (click here to view). All of our rooms have been beautifully renovated and include 40" flat screen televisions, personal recliners for every resident and electric beds for personal comfort. Dining services are cooked to order, ensuring our food is always healthy, fresh, and home cooked. We want nothing but the best for our residents.
